Forget the above. NetworkManager drove me nuts. Totally disabled NetworkManager, set wlan1 up manually as above, working fine and without any NetworkManager confusion/frustration. Forcing the driver to 54M seems to fail (possibly due to signal) but at 12M I'm getting 1.2Mb/sec which seems reasonable.
