My 2 pence: it seems that "sudo update-initramfs -u" was rebuilding the image for the generic kernel, whereas the default on my Thinkpad X40 was the 386 kernel. So I might have fixed it straight away, but I didn't know it! I was fiddling around for ages, and I have no idea which of the steps I took was the right one.

My final configuration has no edits to usplash.conf and none to menu.lst, so maybe just rebuilding the initramfs image is sufficient to fix this, as long as you rebuild the right one (use "-k <version>" to update-initramfs).
