I can confirm that I have the same problem with the below card on karmic 64bit with all latest updates applied as at time of this writing. No matter what I put into my /etc/modprobe.d/alsa-base.conf the only thing that really works is a "sudo alsa force-reload" (sound/pcm levels aren't the culprit either).
