This implies that ureadahead has crashed; when you see the lingering debugfs mount, is the ureadahead process still in ps? What does "status ureadahead say"?

If not, could you attach your dmesg, syslog and messages files (from /var/log), output of the "dmesg" command wouldn't go amiss either.
