The same problem here:
wlan0: AP denied association (code=12)
AP resets its wlan interface (but uptime on AP does not reset) so every other client connected to it gets disconnected too (repeats about every 2 minutes).
HW specs:
Intel WLAN 5100 card
Airlive 5460AP and 5470AP
Disabling network-manager and connecting manually with wpa_suplicant works.
