* uname -a > uname-a.log
* cat /proc/version_signature > version.log
* dmesg > dmesg.log
* sudo lspci -vvnn > lspci-vvnn.log

Also, can you be sure to capture the entire dmesg output only after having attempted to load the module so that it also includes the warning you are seeing.
