Why do you add vesafb to your /etc/initramfs-tools/modules instead of the nvidiafb?
As far as I know vesafb is for intel chips.

Try the following:

sudo vi /etc/initramfs-tools/modules and add fbcon and nvidiafb
sudo update-initramfs -u -k all
