This may be a useful piece of information.

I am using Ubuntu 9.04. My 16GB flash disk sometimes works and sometimes does not. I can see some rules in the behavior, but getting exact results is time consuming.

I can use Low Level Format utility (http://hddguru.com) to reformat the flash disk in Windows. I can then create a new partition and volume on it using either gparted or Windows tools. The flash disk normally works in Ubuntu as fas as I don not store a lot of data on it. Then it stops working and I am getting the messages above.
I am not quite sure what "a lot of data" means. It may be more then 4GB of data, or a lot of files on the disk in total number, but it is not 255 files in a single directory. It does not matter if I store the data on the disk using Ubuntu or Windows. BUT, if I use Ubuntu to store the data, is works perfectly until I remove the disk from the port. Then, when I attach it to the port againg, it is not recognized anymore.
