I'm thinking I've spot the particular USB configuration when the problem is present. It seems to be when copied large amount of data (say 1.9 GiB) from USB using SD-card reader. I've not check this time but it seems that it does not occure on dd of partition.

Reproduced on 2.6.30.4 - I'll check the -rc soon.
