This is not related to hotkey-setup; from /usr/share/doc/hotkey-setup/README.Debian: "Note: This package does not (on its own) cause the hotkeys to do anything. It merely makes them available to userspace programs."
In KDE/kubuntu, this is done using kmilo. This step size is fixed, but you can use CTRL to have a smaller increase/decrease (http://ubuntuforums.org/archive/index.php/t-344667.html).
