This happens to me with Firefox 3.0b2 on Mac OS X (10.5.1).
I do not remember this happening on Firefox 2.
Even worse than asking for the same password 15 times (literally--I keep a lot of windows and tabs open), is that if I don't get rid of the dialogs fast enough they get locked up stop rendering themselves. It seems that sometimes it asks for the master password with a sheet, and sometimes with a dialog box.
When I have both sheets and dialogs open (because I didn't cancel a sheet quick enough) then sometimes a sheet wont render (it's just a blank sheet, password-asking sized). Also subsequent dialogs wont render either.
Once it's locked up I cant get rid of the sheet and so the whole window will not respond to any input. I have to quit firefox and hope that next time it comes up the timing will be slightly different...
