Broken here with x64 12.04 too. It's *extremely* annoying as I have a cheap KVM switch between two computers and every time I switch, I have a broken xmodmap.
Thomas Albright, if you're still reading this, I don't suppose you can post more information on your workaround so that everyone else who runs into this doesn't have to go through an hour of figuring out what to do.
