It occurred while upgrading ubuntu kernel-related packages on June 12th, 2009 at 5:30 PM.
On the page it says that required information is automatically included.

I am using Jaunty 9.04 and the only thing different from other times on my side was that I chose the "3-way merge of menu.lst file (experimental)" option. I tried this option because I was tired of manually changing the menu.lst. (Windows partition information was always erased after I upgrade to a newer kernel.)
