Intrepid Program Launcher widget not displayed

Bug Description

Using Intrepid A3 64 bit Kubuntu (KDE 4.1.0) on AMD 64.
Yesterday I lost the Program Launcher Menu that is normally displayed on the left-hand corner of the task bar (i don't know how).
If I right-clicked on the task bar and 'Add widgets' it seemed that the Application Launcher Menu was still available (as shown by the '-' symbol), it was nowhere to be seen. I then selected the star icon and turned it yellow so that I got a new Application Launcher Menu displayed on the right-hand side of my taskbar.
Today I booted into Kubuntu and the Program Launcher Menu has again disappeared. Yet the add widgets suggests it should be somewhere. In order to run programs I've now dragged a new Program Launcher Menu onto my desktop. See the state of the Add widgets window (attached).

Again the Program Launcher Menu widget has gone AWOL. The number by the Add widgets PLM is up to 3.

I haven't as yet been able to reproduce this in the latest kernel (A5 2.6.27.2).
