Bug Description
The gradebook should not permit teachers to create two activities in the same worksheet with the same name. This will be a particular problem with external activities that get their names from a drop down dialog instead of a text entry box, so accidentally choosing the same name twice is a likely occurance.

We feel that now that we have the label attribute for activities, having duplicate names is no longer a problem.
