Date format ignored when PDF File generated

Bug Description

When you click on the "Printer" button to generate a pdf file the date format
selected in preferences is ignored and default to YYYY-MM-DD Day Name

Oh sorry, found it. It's the calendar thing. Both date and time preferences are ignored in the PDF views.

I think the year format set in the server tab is ignored on the entire system. It appears that only the default option (yyyy-mm-dd) works. For example, when you create a custom demographics field that has a date format, it uses yyyy-mm-dd.
