Firefox icon gone after upgrade to 9.10

Bug Description

When I upgraded from Ubuntu 9.04 to 9.10 the Firefox icon disappeared and was replaced by a gray question mark in the menu and red ring+stroke with a black background at the panel.

It is still gone.

Restarting firefox should fix the menu icons and as for panel icon you may need to change it by hand, more than likely the cause for this is the firefox-3.0 -> 3.5 as default. Please post the output of the following commands:

apt-cache policy firefox-3.0 firefox-3.5 firefox ubufox
lsb_release -a
uname -a

Just a side note:
the package named firefox in Launchpad refers to firefox-2.0

I fixed this bug by reformatting the hard disk and installed the system again. The upgrade from 9.04 to 9.10 didn't work. For me the bug is irrelevant.
