Bug Description

Binary package hint: asterisk

After upgrading one of our Asterisk servers to lucid we found that
even the most simple followme configurations were broken. The
followme app would dial people but when they answered they got dead
air (no prompt).

The following patch fixes that and also some trivial/obvious
cut'n'waste damage around prompts. The patch is an amalgamation of
the following two patches:

  https://issues.asterisk.org/file_download.php?file_id=25455&type=bug
  https://issues.asterisk.org/file_download.php?file_id=25444&type=bug

We're now running with this patch in production and it fixes the
problem for us.
