Bug Description

Maverick Meerkat (10.10) Release Candidate and Final Release
Lucid Lynx (10.04)
Karmic Koala (9.10)

To recreate run Cheese, Ekiga or aMSN (basically anything that will try to use the webcam).

Symptoms:

1. Obviously the webcam does not work.
2. Dmesg output that looks like this:
[ 979.810911] ehci_hcd 0000:00:13.5: force halt; handhake ffffc90000364424 00004000 00000000 -> -110
3. Wireless adapter will immediately disconnect after which NetworkManager will just display 'device not ready'
(this happens when your built-in wireless is an internal USB device)
4. Suspend and/or hibernate _might_ fail if attempted.
5. In some cases inserting a memory stick or other USB storage will not work any more.

Rebooting is necessary to fix the other USB devices.

This bug seems to affect varied models of the Toshiba Satellite line as well as some other notebooks. So far a commonality between the different cases seems to be that they all use Chicony webcams and in cases where the wireless fails, the wireless adapter is also a USB device. I am collecting a list of device ids from other reporters and duplicates to this file: http://www.saunalahti.fi/~amodinos/andreas/bug554263.txt

For me this is a regression since Ubuntu 9.04.

Possible work-arounds:
1) Don't use the webcam: add 'blacklist uvcvideo' to /etc/modprobe.d/blacklist.conf
2) Unblacklist usbmouse and usbkbd by commenting them out (based on bug 435352 comment #36)

Dmesg gives the following messages when running any piece of software that might use the webcam (I've tested aMSN, cheese and Ekiga):

[ 981.120293] wlan0: deauthenticating from 00:18:39:2f:ba:b2 by local choice (reason=3)

I was able to make cheese run by doing a sudo nano /etc/modprobe.d/uvcvideo.conf and writing the following line in there:
options uvcvideo quirks=1

With this, I am able to run cheese once with the web cam working properly. Running cheese again or trying to run any other software has so far resulted in the same behaviour as described earlier.

Writing the line on uvcvideo.conf did exactly as Andreas described.

Had this issue in 9.04, was under the impression that the 2.6.32.2 kernel would fix it (tried that custom kernel with 9.04 per Bug 462336, and webcam worked, but wireless didn't..)

This bug was reported a while ago and there hasn't been any activity in it recently. We were wondering if this is still an issue? Can you try with the latest development release of Ubuntu? ISO CD images are available from http://cdimage.ubuntu.com/releases/ .

If it remains an issue, could you run the following command from a Terminal (Applications->Accessories->Terminal). It will automatically gather and attach updated debug information to this report.

Also, if you could test the latest upstream kernel available that would be great. It will allow additional upstream developers to examine the issue. Hi guys...i have tried to install GUVCWIEV and when i try to use it with my webcam with wi.fi card in funcions all work perfectly...wi-fi don't crash and webcam show me the image...but the problem persist when i use cheese o emesene...anyone have news??

System works fine with SPC900NC camera using pwc driver.

Probably very similar problem on my
Dell (AMD TurionX2 2000MHz, AMD RS485 Northbridge, AMD SB600 Southbridge) and Ubuntu 10.04 64-bit.

Linux version 2.6.32-24-generic (buildd@crested) (gcc version 4.4.3 (Ubuntu 4.4.3-4ubuntu5) ) #42-Ubuntu SMP Fri Aug 20 14:21:58 UTC 2010 (Ubuntu 2.6.32-24.42-generic 2.6.32.15+drm33.5)

After several on/off cycles of the external USB webcam (e.g. I run cheese or skype several times, camera still connected)
camera suddenly stops working (I cannot turn it on again).

It is still listed in lsusb, but is not listed between multimedia devices anymore...

All other connected USB devices continue working,
but it is not possible to connect new USB devices, e.g. flash disk dongle or external HDD,
they are not listed in lsusb and there are no traces of them in kernel log...

I have to reboot the system to get the camera and USB system working again.

Kernel log shows:

I think the problem is triggered by uvcvideo and causes ehci_hcd to crash.

Is somebody from kernel team aware of the problem with ehci?
I think we should have USB system tuned to perfection in 2010, it's very essential!

I have now successfully tested the latest daily build of Maverick LiveCD.
The bug still occurs exactly as described.

Unfortunately I cannot personally test the upstream kernel as the hardware I have available for testing is not mine and is in active use. Could someone else who has the same problem contribute this information? That way maybe we could get the ball rolling on this one...

I think I have narrowed this problem down. It seems as if these criteria are common among bug reports:
a)64-bit processor and/or kernel
b)ati/amd graphics card using flgrx driver
c)usb webcam and usb wireless card
d)only disconnected when trying to ACCESS webcam
e)uvcvideo driver
I believe this is a conflict between the FLGRX driver, UVCVIDEO driver, and Wireless kernel module as a result of a usb conflict. please correct me if you disagree. Thanks.

I think this is a bug with uvcvideo how it handles usb devices.

I have the same problem on my desktop with a SYBA SY-CAM63012 usb webcam. I am able to use the webcam with skype after plugging it in, but it stops working at some point and can not be restarted without powering off the entire machine. Soft reboot (restart) does not clear the problem.

I suspect that the usb controller chip on the motherboard shuts down one or more usb ports, and requires a complete power cycle to reset it's internal logic. I will have to test this further.

Acer Veriton M410. AMD Athlon 64 X2 Dual Core Processor 5000+. ATi Radeon HD 4350.
Ubuntu 10.04 LTS, kernel 2.6.32-27-generic-pae (32 bit), xorg radeon driver (NOT flgrx).

This is the dmesg line that indicates the problem.
    ehci_hcd 0000:00:13.5: force halt; handhake f8248024 00004000 00000000 -> -110

Webcam is reported by lsusb as "0ac8:3420 Z-Star Microelectronics Corp." and skype says it is a "Venus USB2.0 Camera"

I think the list of things that are common between reports is more like
a) Motherboard with AMD SB600 southbridge
b) USB webcam using the uvcvideo driver
c) A USB wireless adapter (or possibly any other USB high speed device that's in active use - see below).
d) USB controller errors, cutting off access to all USB high speed devices including the webcam and wireless, when trying to ACCESS webcam. Connecting new USB devices may also fail.

Everyone also has 64-bit CPUs, but since they're the only kind used with this motherboard chipset that's not terribly surprising. There's also a lot of AMD/ATI graphics cards, but that seems most likely to be a coincidence.

I actually encountered this issue on Gentoo with "046d:0809 Logitech, Inc. Webcam Pro 9000" and a "2040:7070 Hauppauge Nova-T Stick 3" and found this bug report when Googling for solutions, so it's neither Ubuntu-specific nor just restricted to USB wireless adapters.

I also have the same problem with TOSHIBA Satellite A210 PSAFGC-MS408C it crashes when I load cheese in linux ubuntu 9.04 to 10.04 and it crashes the wifi usb built-in totally and only an hard reboot makes the wifi works again but will crash if anything uses webcam and wifi at the same time and doesn't happen on the 2.6.28.0 kernel but anything above that will crash.

Possible workaround found that fixes the crashing problem with the ehci_hcd driver what i did was rmmod ehci_hcd as root then modprobe -a ehci_hcd and the wifi reconnected and the webcam work many times no crashes and all other apps work no problem i think it an timing problem with the ehci_hcd driver is my guess.

(interestingly, the webcam works the FIRST TIME I use it after a kernel upgrade(? - seems this way), but then crashes everytime.)

well after testing pclinuxos 2011 on 2.6.38.8-pclos1.bfs found that bug is still there and what more i did dmesg and got this message "Loading kernel module for a network device with CAP_SYS_MODULE (deprecated). Use CAP_NET_ADMIN and alias netdev-wlan0 instead" and my wifi crashes after that and fix my wifi by doing rmmod ehci_hcd and then modprobe -a ehci_hcd then wifi card of mine is rtl8187B using modprobe -a rtl8187 and my wifi works fine again unless i try using my webcam then the bug comes back

found an bit of solution disable uvcvideo and blacklist it and use Logitech webcam and my wifi doesn't crash anymore and my Logitech webcam works fine

Apparently, some guys found the real cause of the error, which seems to be a hardware issue. Here I post the link:

They (Canonical) can't take care of the problem if it's a hardware issue. Buggy hardware is not unheard of, but it's unfortunately the worst case scenario because in order to fix the problem the hardware either has to be recalled and replaced with non-sucky hardware OR a software fix has to be made for a particular kind of broken hardware. The problem with the latter solution is that it will often create more problems than it solves.

Beware of cheap computers. Ours was a low-end model for just 350 euros (and naturally, too good to be true).

Andrea Modinos, thank you for reporting this and helping make Ubuntu better. This bug was reported a while ago and there hasn't been any activity in it recently. We were wondering if this is still an issue? Can you try with the latest development release of Ubuntu? ISO CD images are available from http://cdimage.ubuntu.com/releases/ .

If it remains an issue, could you run the following command from a Terminal (Applications->Accessories->Terminal). It will automatically gather and attach updated debug information to this report.

apport-collect -p linux <replace-with-bug-number>

Also, if you could test the latest upstream kernel available that would be great. It will allow additional upstream developers to examine the issue. Refer to https://wiki.ubuntu.com/KernelMainlineBuilds . Once you've tested the upstream kernel, please remove the 'needs-upstream-testing' tag. This can be done by clicking on the yellow pencil icon next to the tag located at the bottom of the bug description and deleting the 'needs-upstream-testing' text. Please let us know your results.

well i can still recreate this bug on xubuntu 12.04 i ran cheese and then my wifi killed itself so the bug has never been fixed here my bug report had to save it offline using this: ubuntu-bug linux --save 554263

I can confirm this problem still exists in ubuntu 12.04 and 12.10. Trying to open /dev/video0 forces ehci_hcd to halt. The wireless does not work any more and external usb devices are not recognised either.
